NOTES:
1. Applications to be mailed, e-mailed or faxed to Hamilton County at least 48 hours
before work performed. In case of emergency repairs, application to be submitted
within 48 hours of repair.
2. Road cuts to be repaired according to the design standards of Hamilton County
Engineering Department.
3. This permit is granted under agreement that all repairs necessary by reason of
above work or damage caused thereby shall be completed, and street, road or shoulder
restored to condition satisfactory to the Hamilton County Engineer within sixty
(60) days after date of permit approval. Should it be necessary in the future to
alter, remove, repair or relocate the aforesaid construction or project on account
of any change, repair, improvement or abandonment of said roads or streets, we (utility)
will perform same at our expense on written notice by the County Engineer or authorized
agent. In the event of any failure to do so, we (utility) will pay all costs incurred
by Hamilton County.
